New Patients
We ask that you fill out the new patients information packet, and bring it with you to your appointment. If you are unable to download please arrive early to allow time to fill out this paperwork. Please disclose all medical conditions and oral or topical medications being used both prescribed and over the counter including, but not limited to vitamins and supplements. The consultation appointment is complimentary and you will meet with Dr. Maltais or one of our clinical staff.
Should you choose to purchase the personalized skin care regimen or any of our medical grade products please note that all monies are due at the time of purchase. No appointment can be made without full payment. You can view our policies under home/our practice/policies for further information.
